ก่อนจะมาเป็นนักศึกษาแพทย์ได้ยินคำพูด เช่น "เรียนแพทย์จะได้ช่วยชีวิตคน" หรือ "ตั้งใจเรียนแพทย์จะได้รักษาคนไข้ได้" ไปจนถึง "ความเป็นความตายคนไข้ขึ้นกับเรา"

เราจะทำได้ถึงแค่ไหนที่จะดูคนไข้ได้ทั้งคน ไม่ว่าจะเป็นโรคอะไรและไม่ได้ดูแค่ความเจ็บป่วย

ค่อยๆนึกภาพเบื้องหลังของชีวิตคนป่วยคนหนึ่งมาพบแพทย์เพราะอาการของโรคที่เกิดขึ้นและรบกวนชีวิตประจำวัน การพูดคุยกับคนไข้ การซักประวัติอาการของโรคถือเป็นการซักถามเรื่องราวที่เกิดขึ้นเบื้องหน้า แต่การไถ่ถามถึงเรื่องราวเบื้องหลังนั้น มีความสำคัญไม่แพ้กัน

แต่...มักจะถูกมองข้ามด้วยเวลาอันจำกัดของแพทย์


สำหรับเรื่องราวเบื้องหลังนั้นคือ การรับฟังเรื่องราวประสบการณ์ที่ผ่านมาในชีวิตของคนไข้ ซึ่งบางครั้งสำคัญและอาจเป็นสาเหตุของโรคที่นำคนไข้มาพบแพทย์ เช่นในคนไข้จิตเวช จะต้องถามถึงการใช้ชีวิตที่ผ่านมา

ซึ่งอาจจะเต็มไปด้วยความสิ้นหวังและยากต่อการปลอบประโลม แต่ถ้ามีเวลาการรับฟังและใส่ใจในจุดนี้ จะช่วยในความสัมพันธ์ระหว่างหมอและคนไข้อย่างมาก นำไปสู่การรักษาที่ดีขึ้นหรือคนไข้ในสาขาอื่นๆ

การรับฟังเรื่องราวเบื้องหลังอาจจะนำมาสู่การวินิจฉัยที่แม่นยำ

ก่อนมาเป็นนักศึกษาแพทย์จะนึกถึงคุณค่าในตัวตนของทุกคนเสมอ เพราะทุกคนย่อมมีคุณค่าของตัวเอง การยอมรับและเคารพในตัวตนของผู้อื่นนั้น เป็นสิ่งที่แพทย์พึงระลึกไว้เสมอ แต่การเรียนแพทย์นั้นมุ่งเน้นถึงการวินิจฉัยและรักษาโรคซึ่งซับซ้อนขึ้นทุกปีตามความก้าวหน้าของวิทยาศาสตร์ร่วมกับเวลาอันน้อยนิด


จนบางครั้งคุณค่าในตัวตนของผู้ป่วยจึงเจือจางลงไป เช่นในวอร์ดอายุรศาสตร์มีผู้ป่วยหลายคนที่คิดว่าอาการเจ็บป่วยของเขาสร้างความลำบากแก่ครอบครัวหรือการป่วยเป็นโรครุนแรงทำให้ไม่สามารถเคลื่อนไหว ลองนึกดูว่าถ้าวันนึงไม่สามารถแม้แต่จะกินข้าวเองได้จะรู้สึกยังไง

สิ่งเหล่านี้บั่นทอนกำลังใจและคุณค่าในตัวผู้ป่วยลง จนอาจคิดว่าตนเองนั้นด้อยค่า ไม่อาจตัดสินใจในเรื่องสำคัญใดๆได้อีก

ซึ่งความจริงแล้วผู้ป่วยมีสิทธิในการตัดสินใจเรื่องการรักษา มีสิทธิที่จะเลือกชีวิตในบั้นปลายที่เหลืออยู่ และมีสิทธิที่จะให้ผู้อื่น เข้าใจและเคารพในการตัดสินใจของเขา ซึ่งแพทย์ก็ควรจะพยายามถามถึงความต้องการของผู้ป่วย เน้นว่าผู้ป่วยและไม่ใช่ถามแต่ญาติ

จากนั้นเคารพในการตัดสินใจของผู้ป่วย หรือพยายามทำตามความต้องการที่อยู่ในขอบเขตของเหตุผล เนื่องจากในบางครั้งทางเลือกที่แพทย์เห็นว่าดีหรือญาติคิดว่าดีอาจไม่ได้ดีที่สุดสำหรับคนไข้เสมอไป

นอกจากการเคารพในคุณค่าและการตัดสินใจของผู้ป่วยแล้ว การมองตัวโรคและตัวตนของผู้ป่วยแยกกัน เพราะตัวโรคอาจทำให้สภาพร่างกายและจิตใจเปลี่ยนแปลงไปมาก

สิ่งสำคัญนอกเหนือจากการรักษาคือการใส่ใจคุณค่าและตัวตนเหมือนดังเช่นในยามที่มีสุขภาพร่างกายและจิตใจ
ที่แข็งแรง เพื่อการแนะนำคนไข้สู่การตัดสินใจในเรื่องต่างๆที่เหมาะสมที่สุด เพราะในช่วงเวลาคาบเกี่ยวความตาย ไม่ใช่เวลาที่จะมาตัดสินใจ

ควรที่จะคุยและตัดสินใจเรื่องสำคัญต่อการรักษาและชีวิตไว้ให้เรียบร้อยในขณะที่ยังมีเวลา เพื่อที่จะไม่ให้เป็นการตัดสินใจที่ฉุกละหุกและไม่ได้ไตร่ตรองไว้ก่อน


บ่อยครั้งไม่ได้มีการคุยเพราะเป็นเหตุฉุกเฉิน แต่บ่อยครั้งก็เป็นเพราะว่าแพทย์ไม่มีเวลา ไม่ก็ผู้ป่วยหรือญาติยังตัดสินใจไม่ได้ จะพูดถึงในกรณีที่คุยตกลงกันไว้แล้วเพราะเป็นผู้ป่วยระยะสุดท้าย

ครั้งแรกในชีวิตของ นศพ. ที่จำได้ดี ขณะอยู่เวรที่หอผู้ป่วยหนัก พยาบาลประจำวอร์ดได้แจ้งว่าคนไข้คนหนึ่งไม่มีสัญญาณชีพ "น้อง นศพ.ช่วยไปยืนยันว่าคนไข้ไม่มีชีพจรและแจ้งพี่แพทย์ประจำบ้านด้วยนะคะ" ซึ่งคนไข้คนนั้นเป็นคนไข้ที่ทางวอร์ดได้คุยกับญาติแล้วเรื่องปฏิเสธการกู้ชีพและให้เป็นไปตามธรรมชาติ

สิ่งที่ทำคือขออนุญาตคนไข้ตรงหน้าเหมือนที่เคยทำในทุกๆครั้งและดูการตอบสนองต่อเสียงและใช้หูฟัง ฟังเสียงการเต้นของหัวใจ หน้าอกยังเคลื่อนขึ้นลงช้าๆ

เพราะเครื่องช่วยหายใจไม่ได้มีการหายใจเองและไม่มีเสียงสัญญาณชีพอื่นที่บ่งบอกถึงการมีชีวิต คนไข้เสียชีวิตแล้ว

และสิ่งที่ทำได้ในวันนั้นคือการเดินไปบอกพี่แพทย์ประจำบ้านให้ทางวอร์ดแจ้งการเสียชีวิตกับญาติคนไข้เพราะว่าดูแลคนไข้คนนี้มานาน จึงมีความผูกพัน ถึงรู้ว่าเป็นระยะสุดท้ายและเป็นตัวอย่างที่ดีของการคุยไว้ก่อนจะได้มีการวางแผนการรักษาที่เหมาะสม

แต่ก็อยากให้รอดและกลับมาดีขึ้น

ฉะนั้นที่ว่าแพทย์เป็นผู้กำหนดความเป็นความตายของผู้ป่วยคงจะเป็นคำพูดที่เกินจริง เพราะทุกคนมีสิทธิที่จะตัดสินใจในชีวิตของตัวเอง ยกตัวอย่างว่าถึงจะอยากมีชีวิตแต่แพทย์เป็นคนเหมือนกัน ที่ตั้งใจจะใช้ความรู้ ความสามารถเท่าที่มีเพื่อดูแลคนไข้ และจะพยายามเคารพในคุณค่าและตัวตนของทั้งญาติและคนไข้

แต่ถ้าตัวโรคได้ถึงระยะสุดท้ายแล้ว หรือระบบสาธารณสุขรวมถึงบุคลากรในนั้นได้มาถึงจุดที่ไม่ไหวแล้ว ทุกอย่างจะค่อยๆเจือจางหายไป โดยเฉพาะในสถานการณ์วิกฤติเช่นนี้.
